Pink First, Implant Second: Understanding Why Soft Tissue is the Key to Implant Success

Implant dentistry often celebrates fixtures, drills, and prosthetics—but long-term success is won or lost in the soft tissue. This presentation reframes treatment planning around the peri-implant mucosa: creating a durable biologic seal, stabilizing the margin, and giving implants the best chance for long-term success. We’ll cover phenotype conversion when and how to add keratinized tissue, and how to address different clinical scenarios including immediate and delayed implant placement protocols. Evidence and real cases will show how small “pink” decisions reduce complications—gingival recession, mucositis/peri-implantitis, food impaction, and esthetic failures—across single units and cases in the esthetic zone. 

 Course Objectives:

  • Diagnose soft-tissue risk and stratify implant cases accordingly.
  • Understand the tools of modern periodontal plastic surgery to augment the implant sites (connective tissue or free gingival grafts, and bone grafts) and correct pathology and deformities on adjacent teeth including gingival recession and altered passive eruption.
  • Plan immediate vs. delayed protocols to preserve or augment soft tissue at the time of extraction/implant.
  • Understand the effect of provisionals and customized healing abutment to sculpt the transmucosal profile and support papillae/emergence.
